REQUIREMENTS FOR NEW NAMIBIAN PASSPORT AND EMERGENCY TRAVEL CERTIFICATE
NB: ALL COPIES MUST BE CERTIFIED
1. Completed Passport application form
2. Two passport photos
3. Namibian Birth Certificate or Citizenship Certificate: persons born outside Namibia
4. Namibian ID/Proof of ID application (Both sides of ID/Proof of ID application)
Passports will be printed based on information on ID/Proof of ID application
5. Original passport
6. Marriage Certificate (Also note: Married women who apply with husband’s surname, must apply for change
of surname and signature on ID card at the same time as they apply for new passport - in case the surname on ID card is with previous surname. The form can be requested from the Namibian
Embassy)
7. Divorce Certificate (Women who wish to revert back to maiden surname)
8. Replacement of lost/damaged Passport: Police Affidavit
9. Declaration regarding the loss of a Passport or other Travel Document
10. Proof of payment
Fees: 400 SEK (New passport) + Bank charges: 50 SEK (Foreign payments)
800 SEK (New passport - Double payment) if the current passport is valid for more than 6 months once the application is received by the Ministry of Home Affairs, Immigration, Safety and Security + Bank charges: 50 SEK (Foreign payments)
800 SEK (Damaged or lost passport) + Bank charges: SEK 50 (Foreign payments)
If the new passport - once issued - is forwarded to applicant by Registered mail:
Mailing fee to be paid when applying: 200 SEK

1. Completed Passport application form
2. Two passport photos
3. Namibian Birth Certificate or Citizenship Certificate: persons born outside Namibia
4. Namibian ID/Proof of ID application (Both sides) (16 years and above)
5. Marriage Certificate
6. Parents Documents e.g. Birth or Citizenship Certificates and ID (applicants under the age
of 16)
7. Applicant’s signature (13 years and above)
8. Proof of payment: Fees: 300 SEK + Bank charges: SEK 50 (Foreign payments)
If the Emergency Travel Certificate - once issued - is forwarded to applicant by Registered mail: Mailing fee to be paid when applying: 200 SEK
NB: A COMMISSIONER OF OATHS MUST CERTIFY THE APPLICATION
NB: Bank charges are to be paid for foreign payments (Payments made from Bank accounts abroad).
TWO PASSPORT PHOTOS IN COLOUR:
White background, shoulders square to camera, both ears visible, forehead/shoulders/ears must not be covered, no visible shadow, no head gear, no eyeglasses, no uniform, no smile, no earrings for men, no eyebrow/chin/nose rings for all applicants, requirements as per International standards
